  <abstract>To determinine the prevalence of septal perfusion defects in patients with left and right bundle branch blocks, 18 patients with LBBB and 49 patients with RBBB who underwent myocardial perfusion imaging (MPI) were studied. There were 4 (22.22%) LBBB patients with septal perfusion defects. Two of the 4 had coronary angiography, one was normal while the other showed 40-50% stenosis in the left anterior descending (LAD). Five (10.20%) of the RBBB patients showed septal perfusion defects. None of whom had coronary angiography. Whereas previous studies suggested a high percentage of false positive results in patients with LBBB and some similar reports in RBBB, this study demonstrates a low prevalence of septal perfusion defects (22.22% in LBBB and 10.20% in RBBB) in an unselected population presenting for (MPI). Therefore MPI remains a dependable tool for evaluating patients with bundle branch blocks.</abstract>
