520 3 _aFifty eight pregnancies complicated with congenital heart disease who delivered at the Department of Obstetrics and Gynecology, Philippine General Hospital from 1988-1992 were reviewed. The mean age and parity did not differ from the general obstetrical population. 93.1% had prenatal consults and the majority were in functional class 1 and 2. 79.3% of deliveries were vaginal. Caesarean section was done mainly for obstetrical indications. There is a 10.3% and 39.6% risk of maternal and fetal complications respectively. The data suggests that nulliparas have a higher incidence of maternal complications. A higher incidence of cardiac and non-cardiac complications in patients with congenital than acquired heart disease was noted. Asphyxia is seen more commonly in patients with acquired than congenital heart disease. In the study group, a trend towards an increase in fetal complication with increasing age and parity is seen. An increase in maternal and fetal complications with increasing functional class is also seen. In view of the above data, it is suggested that close prenatal and intrapartal care should be given to pregnant women with congenital heart disease.
