It is in this context that family physicians are dealt with a unique mission of not just caring for the patient, but for his/her family with the involvement of the community as well. Today, we present a patient whose caregiver cares for two patients at the same time. The family enters a homecare program where a healthcare team carefully dissects and manages their biological, psychological and social challenges. Here, we will also see how family physicians can help coordinate the actions of community members to produce fruitful results.
